What is BCG treatment?

BCG (Bacillus Calmette-Guérin) is an immunotherapy used to treat non-muscle-invasive bladder cancer (NMIBC). It contains a weakened strain of bacteria related to the tuberculosis vaccine. Rather than directly killing cancer cells, it stimulates your immune system to attack cancer cells in the bladder.

Who is BCG recommended for?

BCG is commonly recommended for patients with: High-grade Ta bladder cancer T1 bladder cancer Carcinoma in situ (CIS) Patients at intermediate or high risk of recurrence after tumor removal (TURBT) It is generally not used once bladder cancer has invaded the muscle.

How is BCG given?

BCG is placed directly into the bladder through a small catheter (intravesical therapy). The medication stays in the bladder for about 2 hours, then you urinate it out. No surgery or anesthesia is usually required.

How many BCG treatments will I need?

A typical BCG schedule includes:

  • Induction therapy Once a week for 6 weeks
  • Maintenance therapy (if appropriate)
  • Additional treatments over 1–3 years, often given as 3 weekly treatments at intervals (for example, at 3, 6, and 12 months, with some patients continuing longer)

Does BCG treatment and therapy cure bladder cancer?

BCG (Bacillus Calmette-Guérin) treatment and therapy can:

However, regular cystoscopy and follow-up are still essential because bladder cancer can return.

What are potential serious side effects from BCG?

Contact your healthcare team immediately if you develop:

  • Fever over 38.5°C (101.3°F) lasting more than 24–48 hours
  • Chills or shaking
  • Difficulty breathing
  • Severe pain
  • Heavy bleeding
  • Inability to urinate
  • Signs of a widespread infection (persistent fever, severe weakness)

Although rare, BCG can cause a serious systemic infection that requires prompt treatment.
